This medication is used for the primary treatment of hypercholesterolemia (high cholesterol) or hyperlipidemia (high levels of lipids/fats in the blood, including cholesterol and triglycerides). Pitavastatin is a statin, or HMG-CoA reductase inhibitor. It is a lipid-lowering drug that reduces cholesterol production by the liver. By lowering “bad” cholesterol (LDL) and triglycerides and raising “good” cholesterol (HDL), it reduces the risk of cardiovascular disease and helps prevent strokes and heart attacks. Clinical studies in 2026 continue to show that Pitavastatin has a neutral effect on blood glucose, making it a preferred option for patients with or at risk for diabetes. Furthermore, it has a unique metabolic profile; unlike many other statins, it is not primarily metabolized by CYP3A4, resulting in a significantly lower risk of drug-drug interactions. Treatment works best when combined with a healthy diet and other lifestyle changes, including exercise, weight loss, and smoking cessation. These tablets are taken once daily, at any time, with or without food. Other cholesterol-lowering medications include Crestor, Lipitor, and Zetia.

In a study (Efficacy and safety of pitavastatin in patients with high cardiovascular risk: A meta-analysis of randomized controlled trials – ScienceDirect, 2024), researchers confirmed that pitavastatin effectively reduces LDL-C and cardiovascular risk while maintaining a superior safety profile. The study found that “pitavastatin showed a neutral effect on glucose metabolism and did not significantly increase the risk of new-onset diabetes mellitus (NODM) compared to other high-potency statins,” reinforcing its role as a preferred treatment for patients with metabolic concerns.

Manufacturer Information

We currently have the brand-name product from Recordati Pharma in Turkey and Kowa Pharma in the United States. It is also known as Alipza.

The generic equivalent, pitavastatin, is available and manufactured by various pharmaceutical companies globally.

Livalo is available in strengths of 1mg, 2mg, and 4mg tablets.

The information below lists potential side effects and risks of Livalo. It is crucial to discuss these with your doctor or pharmacist.

Common Side Effects – The following are common side effects. Contact your doctor if you experience these side effects and they are severe or bothersome.
• Back pain
• Constipation
• Diarrhea
• Muscle pain (myalgia)
• Pain in arms or legs

Other Potential Side Effects: (less common) – These effects are reported less frequently but can still occur:
• Abdominal discomfort
• Dizziness
• Fatigue
• Headache
• Insomnia
• Joint pain (arthralgia)
• Nausea

Serious Side Effects - These are rare but potentially severe. Seek your doctor or call medical emergency services immediately if any of the following occur:
• Hypersensitivity Reactions: Severe rash, hives, or swelling of the face, lips, or throat, which requires immediate medical attention.
• Liver Dysfunction: Signs of liver strain, including dark urine, severe upper stomach pain, or yellowing of the eyes and skin (jaundice).
• Memory Issues: Rare reports of confusion or cognitive impairment that typically resolve upon discontinuation of the drug.
• Rhabdomyolysis: A rare but severe muscle breakdown leading to kidney damage; symptoms include extreme muscle weakness, fever, and tea-colored or dark urine.
• Unexplained Muscle Pain: Persistent, unexplained muscle tenderness or weakness, especially if accompanied by a high fever or unusual tiredness.

The most significant clinical concern with Livalo therapy is the risk of muscle-related issues, ranging from mild muscle pain to a rare but life-threatening condition called rhabdomyolysis. To manage this risk, patients must report any unexplained muscle pain, tenderness, or weakness to their healthcare provider immediately. This risk is notably higher in patients who are elderly (65 and older), have untreated hypothyroidism (underactive thyroid), or have pre-existing kidney disease.

Additionally, this medication has a strict interaction profile. It is contraindicated with cyclosporine because it can cause dangerously high blood levels of pitavastatin. Although it has a lower risk of interactions with many common drugs since CYP3A4 does not primarily metabolize it, caution is still required when it is combined with certain antibiotics (such as erythromycin or rifampin) or colchicine, as these can increase the risk of muscle damage. It is not recommended during pregnancy or while breastfeeding because it may cause fetal harm and can pass into breast milk. Before starting therapy, you must disclose all medications you are taking, including over-the-counter (OTC) drugs, vitamins, and herbal supplements.

Dietary and Lifestyle Factors

During treatment, certain lifestyle adjustments are necessary to ensure the medication's safety and effectiveness. Unlike many other statins that must be taken in the evening for optimal results, it can be taken at any time of day, as long as it is taken consistently at the same time each day. To keep stable drug levels in your system, do not skip doses or change your timing without consulting your doctor.

Regarding diet, this medication is highly effective, but it works best when paired with a heart-healthy diet low in saturated fat and cholesterol. However, it is less likely to interact with other medications than other statins. Unlike some other statins, it does not have a known significant interaction with grapefruit or grapefruit juice because CYP3A4 does not primarily metabolize it.

Furthermore, because excessive alcohol consumption can strain the liver and increase the risk of side effects, you should limit alcohol intake while on this therapy. Finally, regular physical activity and weight management are crucial components of your treatment; however, you should avoid sudden, extreme bursts of strenuous exercise without proper conditioning, as this can increase the risk of muscle injury while taking a statin.
