Co-administration with strong CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin) or strong CYP3A4 inducers (e.g., rifampin, St. John’s Wort) is strictly prohibited because these agents can cause dangerously high or low plasma concentrations of lurasidone. The use of strong CYP3A4 inhibitors is contraindicated. When combined with moderate CYP3A4 inhibitors, such as grapefruit juice or certain doses of diltiazem, the medication dose should be adjusted—usually not exceeding 40 mg daily—under strict medical supervision.
Additionally, clinicians should be aware that it may cause orthostatic hypotension, so caution is advised in patients with known cardiovascular or cerebrovascular conditions.

To guarantee proper systemic absorption, always take this medication with a meal containing at least 350 calories. If you have a history of blood pressure issues, rise slowly from a sitting or lying position to prevent dizziness or falls caused by orthostatic hypotension.
Excessive alcohol intake should be avoided, as it can worsen drowsiness and greatly impair cognitive function. Since this medication may interfere with the body's ability to regulate core temperature, it is important to stay well hydrated and avoid excessive heat or vigorous physical activity. Lastly, because it may cause blurred vision or drowsiness, you should avoid driving or operating heavy machinery until you are sure the medication does not impair your alertness or vision.
