Qternmet XR contains dapagliflozin, saxagliptin, and metformin hydrochloride. It is used along with diet and exercise to improve blood sugar (glucose) control in adults with type 2 diabetes. It is not for people with type 1 diabetes and not for people with diabetic ketoacidosis (increased ketones in blood or urine). This medication is only for people currently taking metformin. It is not known if Qternmet XR is safe and effective in children younger than 18 years of age. Take the medication exactly as prescribed by your doctor. Take Qternmet XR by mouth 1 time each day in the morning with food. Swallow the tablets whole, do not cut, crush or chew the tablets. Stay on your prescribed diet and exercise program while taking this medication.

Stop taking Qternmet XR and call your doctor at once if you have one of these serious side effects:

• Bladder cancer
• Dehydration
• Heart failure
• Inflammation of pancreas
• Ketoacidosis (nausea, tiredness, vomiting, trouble breathing, abdominal pain)
• Kidney problems
• Lactic acidosis
• Low blood sugar
• Skin reactions
• Vaginal yeast infection

Less serious side effects include upper respiratory tract infection, abnormal amount of fats in blood, urinary tract infection. Please call your doctor if you have any further concerns or questions.

Avoid drinking alcohol very often or drinking a lot of alcohol in a short period of time (“binge” drinking). It can increase your chances of getting serious side effects. Do not take Qternmet XR if you are allergic to dapagliflozin, saxagliptin, metformin or any other ingredients in the medication, have moderate to severe kidney problems, are on dialysis, have a condition called metabolic acidosis or diabetic ketoacidois. Qternmet XR should not be used in children. The safety of its use in children has not been determined.
